
When is it appropriate to dismiss/terminate an employee after multiple negative evaluations? And how many evaluations would it take to do so?

Good questions, you may wish to consult your organization's HR and its policies regarding the termination process. Let me say though that dismissing an employee should not be a surprise and that negative evaluations should also include a coaching and planning element where expectations and parameters regarding performance and improvements are clearly identified.
