Upon exiting and needing documentation of credits, is this a simple form developed by the institution?

The school should have a transcript form and place that shows official information signed by an authorized official of school.

I really think this is an important task that should be completed by the school. All the schools I have been associated with due have this process in place. The student is responsible for his/her own information gathering about the transferability of courses and the schools that receive those credits.
